    Unexplained Infertility

Infertility is called unexplained when a couple has been having regular intercourse without conceiving, and standard tests (ovulation, fallopian tubes, uterus and semen analysis) are all normal. It accounts for about a quarter of infertile couples. Several factors are thought to contribute, and research continues to uncover new causes.

    Weight and fertility

People with a high body mass index (BMI) are more likely to have difficulty conceiving than those of normal weight. Women with obesity are more prone to ovulation and hormonal disorders, respond less well to fertility drugs and often need higher doses. If pregnancy happens, the risks of miscarriage, high blood pressure, gestational diabetes and preterm birth are higher. Losing even 5–10% of body weight can restore ovulation and improve the chance of pregnancy. Being significantly underweight can also stop ovulation.

    Stress and fertility

Infertility itself is stressful, and pressure from family and society adds to it. In men, psychological factors can cause erectile problems, premature ejaculation or absent ejaculation. In women, severe stress can disturb ovulation, and anxiety can cause vaginal muscle spasm (vaginismus) that makes intercourse painful. Stress is rarely the only cause of infertility, but support and counselling are an important part of care.

    Treatment options

Current guidelines recommend a stepwise approach based mainly on the woman’s age and how long the couple has been trying: a period of expectant management for younger couples, ovarian stimulation with intrauterine insemination (IUI), and IVF, which offers the highest pregnancy rate per cycle and also reveals problems with egg or fertilization that standard tests cannot detect.
